As the care and acuity of residents have increased in the past year, the need for meaningful interviews and accurate functional assessments takes on more importance for Quality of Life in our facilities. This program is designed to review core MDS item sections and identify high-risk assessment areas that are frequently missing the in-depth information necessary for setting goals and objectives for optimal resident wellness. The program will also address how utilizing EMR modules in facilities can improve and expedite assessment information. The goal and objectives of the presentation are to provide the foundation and intent of the MDS items to ensure accuracy and capture of information necessary to meet the requirements for appropriate reimbursement and the individualized plan of care for the resident.

Care planning remains high on survey deficiency citations in the many components of identification of the individualized needs of residents. Over the past 30 years, skilled nursing facility regulations have increased requirements for care planning associated with the initial Minimum Data Set and Care Area Assessments. Care planning is a significant component in providing quality care to individuals admitted for both short and long-term admissions. With the increasing availability of EMR, facility staff is relying on software to review and analyze resident problems instead of personally assessing and communicating with the individual and their families/significant others to determine personal preferences, goals for treatment, and discharge planning. The goal and objectives of this program are designed to review the multi-components of assessment, identification of resident strengths and challenges to formulate a multi-disciplinary, meaningful, realistic, and individualized plan of care.
